I have posted an interactive form you can use to contact me with your concerns. You can download, fill out, and return it to me by email if you have a copy of Adobe Reader 7 or higher. (Adobe Reader is available as a free download from Adobe Systems.)

Here's how it works.
1. Download the form by right clicking this link. (If you run Windows, right click and choose "save target as." If you run an Apple operating system, command click and choose "download linked file" from the contextual menu that pops up, or right click if you're using a two button mouse.)
2. Open the form in Adobe Reader and complete the top of the form and Step 1.
3. Save the form.
4. E-mail the form to me by clicking on the "Submit" button at the bottom of the form.
That's it. I'll be in touch.

Here are some tips: Once you place your cursor in and complete the Name field, hitting the tab key moves the cursor from field to field. The text fields in the "Steps" will automatically become scrolling boxes if the text you enter exceeds the space in the static box. And, remember to save the form before sending it to me.
